For removal of eyelid margin tumor, full thickness pentagonal excision and direct closure is generally being used in order to avoid postoperative notching of the eyelid margin. However full thickness pentagonal excision results in not only a vertical scar on the lid margin as well an esthetic proolem due to long vertical scar, particularly after wide lid excision.
We conceived that excision of skin and remaining lamellas of eyelid in seperate direction is ideal for resection of lesions involving eyelid margin in terms of prevention of vertical scar and marginal notching. Full layers of eyelid excluding skin were excised in pentagonal fashion, and remaining skin was excised horizontally parallel to the skin crease.
1. Avoid postoperative lid marginal motching.
2. Prevent vertical scar resulted from pentagonal excision
3. Easy managing of dog ear which may be produced on lower end of the vertical skin incision. We applied this concept to two patients having benign lower eyelid lesions with functionally and aesthetically good results.
